Printing instructions: Set the page size to 100%, then print out all odd pages first, then put those pages back into your printer and print the even pages on back of those, or you can just print page 2 of the pdf file (which contains "Page 15" of the booklet) for the application....OR simply sign up online at the site below.

If you print that out and mail it, be sure to write my ID # V5188 on page 15 where it says "Sponsor #"  (but you'll probably join online because its faster and easier) http://www.flalottomagic.net/?sponsor=V5188

If you have any questions, such as if you're not from USA and you don't have a social security number, you can email flalottomagic@aol.com about what number to use for your ID #.  (They use the last 4 digits of social security number for your ID, with your last initial, and it's needed from USA residents if you earn over $600 per year)
